We examined the utility of corticosteroids in ARDS patients with poor oxygenation and no identifiable respiratory failure mechanisms. We studied all patients with ARDS who were intubated and mechanically ventilated in our ICU over a 16-month period. We evaluated demographics, hemodynamic and respiratory parameters, daily FiO2, hemodynamic data, PaCO2, serum cortisol and adrenocorticotropic hormone (ACTH) data, and information on corticosteroid use. Twenty-four patients met the criteria for ARDS. Mean age was 51.2 +/- 1.2 years (mean +/- SEM). Eight (33%) patients received methylprednisolone at a mean daily dose of 80 mg for a mean of 13.2 +/- 2.5 days. The mean total dose of methylprednisolone for these eight patients was 1,456 mg. Sixteen patients did not receive methylprednisolone. Sixteen patients (66%) died. Oxygenation was improved significantly after corticosteroid treatment in those patients who received methylprednisolone (PaO2/FiO2 [mean +/- SEM], 271 +/- 81 to 537 +/- 96; p = 0.019). This was not seen in the non-corticosteroid-treated patients. Cortisol levels were below the normal range in 11 of 14 patients (79%) who were treated with methylprednisolone.
